Job Description:
Role: Support worker (female candidates only)
Contract: Temp on going
Location: Coventry
Caseload: Max 11 (6 men/5 women)
Hours; 8-5pm 4 days, 11-8pm 1 day.
As a Support Worker, you will play a key role in helping individuals experiencing homelessness to achieve positive life changes and move towards more sustainable lifestyles. You will work collaboratively as part of a supportive team to meet contractual and quality outcomes.
Key Responsibilities
- Facilitate regular one-to-one meetings and client participation activities
- Develop, review, and update individual support plans within required timeframes
- Support individuals to achieve agreed goals, including maximising benefits and managing personal budgets
- Carry out assessments and risk assessments using a person-centred, trauma-sensitive approach
- Coordinate with colleagues to deliver effective in-house support services
- Signpost and refer to external services where additional specialist support is required
- Maintain accurate and timely records using a web-based client management system
- Work in partnership with external agencies such as housing services, health professionals, and community support teams
- Ensure all practice complies with safeguarding, equality, and health & safety policies
- Participate in duty management rotas as required
- Engage in training, supervision, and ongoing professional development.
