Job description

Are you an experienced Adult Social Worker looking for a role that offers greater flexibility, a supportive management team and a healthier work-life balance?

This is an excellent opportunity to join a well-supported Adult Social Care team working with older adults and people with mental health needs. You’ll have the flexibility to work from home for up to three days per week, while remaining part of a collaborative team focused on high-quality, strengths-based practice.

The service understands the demands of frontline social work and is committed to ensuring practitioners feel trusted, supported and able to manage their workload effectively.

The role

As an Adult Community Social Worker, you will work with adults aged 18+ to promote independence, achieve positive outcomes and ensure the right care and support is in place.

Your responsibilities will include:
 

  • Managing a community-based caseload of adults with complex needs
  • Undertaking mental capacity and best-interest assessments where required
  • Supporting adults, families and carers to identify the outcomes that matter most to them
  • Working closely with health professionals, care providers and voluntary-sector partners
  • Safeguarding adults at risk and contributing to timely hospital discharges
  • Maintaining accurate case records and delivering person-centred, legally literate practice

What we’re looking for
 
  • Qualified Social Worker
  • Current Social Work England registration
  • Experience in Adult Social Care
  • Experience supporting older adults, people with mental health needs, or in a related community service
  • Sound knowledge of the Care Act 2014, Mental Capacity Act and adult safeguarding practice
  • A strengths-based, person-centred approach and confident multidisciplinary working skills
