Location: Nottingham
Contract: Full-time, 37 hours per week
Working Pattern: 3 days face-to-face
Are you an experienced Social Worker looking for a rewarding role within a supportive and fast-paced environment? We are seeking one skilled practitioner to join a dynamic team in Nottingham, focusing on hospital-based work and Discharge to Assess (D2A).


🔍 About the Role

You will play a key part in supporting safe and timely hospital discharges, carrying out high-quality assessments, and working collaboratively across health and social care systems.


✅ Key Requirements

• Qualified Social Worker (registered with Social Work England)
• Strong experience in assessment work
• Solid knowledge of Discharge to Assess (D2A) pathways
• Experience working within hospital settings
• Proven ability in multi-agency working
• Good understanding of Continuing Healthcare (CHC) processes
• Full UK driving licence, access to a vehicle, and willingness to travel both within and outside the Nottingham area
